Hydration Isn't Moisture: Humectants, Occlusives & Emollients

Scan the back of almost any bottle of moisturiser and you will find a mix of oils, waters, and butters that all claim to hydrate or nourish, but these ingredients don't all do the same thing. Some pull water into the skin, others seal it in, and some simply smoothen the skin. For kids with dry, sensitive or atopic skin, this distinction between what ingredient does what matters a lot more as their skin not only lacks moisture, but also struggles to hold onto it.
If you spend bedtime applying cream to your child's skin, only to find the same dryness and tightness back by morning, you already know that "just moisturise more" is not always the fix it sounds like. Reapplying more of the wrong type of ingredient will not help their skin hold onto moisture any better, which is why it is worth spending some time to understand what each ingredient can actually do for your child's skin.
Fundamentally, healthy skin relies on its outermost layer to act as a barrier to hold water in and keep irritants out. However, in dry or atopic-prone skin, this barrier usually doesn't function as well, which is why water tends to escape more easily through the surface, a process also knows as transepidermal water loss (TEWL).
Research on children with atopic dermatitis consistently shows this pattern. Other studies suggest that this barrier weakness can even show up before visible symptoms do. A study published in The Journal of Allergy and Clinical Immunology: In Practice followed infants from birth and found that babies who went on to develop atopic dermatitis already showed significantly higher water loss from their cheek skin, months before any dryness was observed.
All of this is part of why dry or atopic-prone skin can feel tight, rough, or itchy again so soon after moisturising, because the underlying issue is more than just "not enough moisture". It stems from a barrier that is fighting to retain moisture. Clinical guidelines for eczema and atopic dermatitis documented in The Journal of the American Academy of Dermatology therefore treat regular, well-chosen moisturising as a core part of caring for atopic skin.
When looking for moisturisers, you may come across the terms 'hydration' and 'moisture' being used interchangeably. However, the two actually have two fundamentally different roles in skincare. In layman terms, hydration refers to water being pulled into the skin while moisture refers to the oils that help keep it there. This concept maps closely onto the three ingredient categories rooted in science that dermatologists actually work with: humectants handle hydration, while emollients and occlusives handle moisture.
These are the compounds that draw water into the outer layer of our skin, either from deeper layers or from the surrounding air. Some common examples of humectants include glycerin, hyaluronic acid, urea (at low percentages), and aloe vera. They are excellent at boosting hydration levels, but on their own, they do little to stop that moisture from evaporating again, especially in air-conditioned or drier environments.
These compounds form a physical layer on the skin's surface to slow water loss and seal moisture in. Some of the most effective examples include petrolatum (petroleum jelly), dimethicone, and mineral oil. Petrolatum at just 5% concentration can reduce water loss by around 99%. This is why many dermatologist-recommended creams for very dry or eczema-prone skin rely heavily on occlusives. However, the trade-off is that their heavier texture can often feel greasier or slower to absorb into the skin.
These are the compounds that help soften and smoothen the skin by filling the tiny gaps between skin cells, which improves how the surface feels and looks. Plant oils and butters such as shea butter, sweet almond oil, apricot kernel oil, and squalane fall into this category. They tend to absorb faster and feel lighter than strong occlusives, but on their own they are generally less effective at preventing water loss over many hours.
It is important to realise that no single category does the whole job. A moisturiser built only on humectants can actually increase water loss on a compromised barrier because it draws moisture to the surface without anything to hold it there. A product that is almost pure occlusive holds water extremely well but can feel heavy and unpleasant, especially on an active child in a humid climate. The reality of having dry, sensitive or atopic skin is that the skin's condition often moves through phases, from itchy flares to feelings of tightness after a shower.
